Please bring completed forms along with insurance information, a photo ID, referral paperwork if you have it, and any prior imaging or reports to your scheduled appointment.

Our image-sharing tool lets you upload and share X-rays and other medical imaging with On Pointe Orthopaedics before your visit, so Dr. Goff can review them ahead of time.

Payment information
We accept most major insurance plans and self-pay. Call our office at 321-321-6551 and we will verify your coverage before your visit, so there are no surprises.
Patient portal
